What is T-Folio?
T-Folio is a free transition portfolio tool for high school age youth with disabilities. It is designed to guide youth in exploring, identifying, and planning for their desired post-school goals. This tool represents a compilation of activities and products showcasing each youth’s ongoing transition process. It is their individual story of who they are, where they have been, and where they are headed after high school.

This tool was designed in Washington state through the collaboration of the Center for Change in Transition Services (CCTS) and the Division of Vocational Rehabilitation (DVR). It aligns with both IEP transition planning and DVR's Pre-Employment Transition Services so that both educators and agency personnel will find it useful in working with youth.
Getting Started
T-Folio Training (PowerPoint)
  • Get oriented to the T-Folio with a quick training by clicking the link above.
  • Each unit is made up of lessons that contain a lesson guide and at least one activity.
  • The lesson guides can be accessed from the lesson landing pages and either followed online or saved as a PDF/printed.
  • The activities can either be filled in online and then saved as a PDF or printed once complete, or they can be printed first and then completed with pen or pencil.
  • Youth can keep track of their documents using the T-Folio Tracker (Word document). The Tracker is introduced in Lesson 1 of the Student-Centered Introduction.
Important!
T-Folio will not store any of the youth's work or information. Users should save their work by "printing to PDF" at the end of each activity:
  • Click on the print button and then choose "Save to PDF" from the printer menu.
  • Make sure youth know to save their work in a designated folder organized by unit and lesson number.
  • Once the form is saved, it can also be printed and added to a physical folder if desired.
